目的

探讨局部晚期头颈部肿瘤患者急性放射性口炎应用捷克信联合抗辐喷的短期疗效。

方法

选择广州医科大学附属肿瘤医院于2020年8月至2021年8月局部晚期头颈部肿瘤所致急性放射性口炎患者102例,依据随机数字表法分为观察组(n=51)与对照组(n=51)。观察组男32例,女19例,年龄(56.32±7.86)岁;对照组男34例,女17例,年龄(55.87±8.39)岁。两组均给予剂量模式为6 996 cGy/33 f,每周5次的局部晚期头颈部肿瘤调强放射治疗。对照组采用抗辐喷;观察组采用捷克信联合抗辐喷。两组患者在放疗结束后7 d评估疗效。比较两组临床短期疗效,治疗前后急性放射性口腔炎分级、疼痛数字评分标准(NRS)评分、口腔评估指南(OAG)量表评分、唾液白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平变化。

结果

观察组治疗7d的短期疗效总有效率为84.31%,对照组为66.67%,差异有统计学意义(P<0.05)。两组治疗后急性放射性口腔炎严重程度较治疗前降低(P<0.05);观察组治疗后急性放射性口腔炎严重程度低于对照组(P<0.05)。两组治疗后NRS评分、OAG评分、唾液IL-6和TNF-α较治疗前降低(P<0.05);观察组治疗后NRS评分、OAG评分、唾液IL-6和TNF-α均低于对照组,分别为[(2.85±0.45)分 vs (3.76±0.63)分,P<0.001]、[(9.20±1.05)分 vs (12.76±1.96)分,P0.001]、[(13.85±2.97)ng/L vs (21.08±4.43)ng/L,P<0.001]、[(28.49±5.24)ng/L vs (39.96±6.56)ng/L,P<0.001]。

结论

局部晚期头颈部肿瘤患者急性放射性口炎应用捷克信联合抗辐喷的短期疗效优于单独抗辐喷的疗效,可减轻疼痛,改善口腔情况。

Objective

To investigate the clinical efficacy of Jeksung combined with anti-radiation spray for radiotherapy-induced oral mucositis in patients with radiotherapy-induced oral mucositis.

Methods

From August 2020 to August 2021, 102 patients with radiotherapy-induced oral mucositis caused by locally advanced head and neck tumors admitted to The Affiliated Cancer Hospital of Guangzhou Medical University were selected. According to the random number table method, they were divided into either an observation group (n=51) or a control group (n=51). The observation group included 32 males and 19 females, with an average age of (56.32±7.86) years. The control group included 34 males and 17 females, with an average age of (55.87±8.39) years. Both groups were treated with intensity modulated radiotherapy (6996 cGy/33 f, 5 times a week) for locally advanced head and neck tumors. The control group was treated with anti-radiation spray, and the observation group was treated with Jeksung combined with anti-radiation spray. The efficacy of the two groups was evaluated 7 days after the end of radiotherapy. The short-term clinical efficacy was compared between the two groups. Radiotherapy-induced oral mucositis grade, numerical rating scale (NRS) score, oral assessment guide (OAG) score, and salivary interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were compared between the two groups before and after treatment.

Results

The total effective rate of the observation group was 84.31%, which was significantly higher than that of the control group (66.67%; P<0.05). After treatment, the severity of radiotherapy-induced oral mucositis in both groups was lower than that before treatment (P<0.05). The severity of radiotherapy-induced oral mucositis in the observation group was lower than that in the control group (P<0.05). NRS score, OAG score, and salivary IL-6 and TNF-α in the two groups were decreased after treatment when compared with those before treatment (P<0.05). NRS score, OAG score, and salivary IL-6 and TNF-α in the observation group were lower than those in the control group after treatment [2.85±0.45 vs 3.76±0.63, P<0.001; 9.20±1.05 vs 12.76±1.96, P<0.001; (13.85±2.97) ng/L vs (21.08±4.43) ng/L, P<0.001; and (28.49±5.24) ng/L vs (39.96±6.56) ng/L, P<0.001, respectively].

Conclusion

The short-term curative effect of Jeksung combined with anti-radiation spray for patients with locally advanced head and neck tumors with radiotherapy-induced oral mucositis is better than that of anti-radiation spray alone, which can relieve pain and improve oral condition.
