二甲双胍、来曲唑联合地屈孕酮治疗多囊卵巢综合征合并不孕症的效果评价

目的

探讨二甲双胍、来曲唑联合地屈孕酮治疗多囊卵巢综合征（PCOS）合并不孕症的临床效果。

方法

将北华大学附属医院妇产科门诊就诊的230例PCOS合并不孕症患者随机分为对照组（n=115）、观察组（n=115）。对照组患者给予来曲唑治疗，观察组患者给予二甲双胍、来曲唑联合地屈孕酮治疗。超声检查确认2组患者的卵泡直径，抽血确定血清雌二醇（E₂）、促卵泡成熟素（FSH）、促黄体生成素（LH）以及睾酮（T）的水平，并判定治疗效果。

结果

不同时间点观察组患者卵泡直径较对照组明显增大（P<0.01）；2组患者血清内分泌激素、E₂、FSH、LH、T变化具有统计学意义（P<0.01）；对照组总有效率为86.96，观察组总有效率为93.91，差异具有统计学意义（P<0.05）。

结论

PCOS合并不孕症患者给予二甲双胍、来曲唑联合地屈孕酮干预，治疗效果较单药来曲唑更为明显，可以有效改善患者卵泡直径，纠正患者血清内分泌激素指标，促进患者妊娠可能。

Objective

To evaluate the efficacy of letrozole combined with metformin and dydrogesterone in the treatment of polycystic ovarian syndrome (PCOS) with infertility.

Methods

A total of 230 patients with PCOS complicated with infertility who visited the obstetrics and gynecology clinic of Affiliated Hospital of Beihua University were randomly divided into a control group (n=115) and an observation group (n=115). Patients in the control group were treated with letrazole alone, and those in the observation group were treated with letrazole combined with metformin and dydrogesterone. Ultrasound was used to confirm the follicle diameter in the two groups, blood tests were performed to determine the levels of serum estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), and testosterone (T), and the therapeutic effect was assessed.

Results

At different time points, the follicle diameter of patients in the observation group significantly increased compared to that of the control group (P<0.01). The changes in serum E2, FSH, T, and LH levels were statistically significant in both groups of patients (P<0.01). The total effective rate in the control group was 86.96%, which was signifciantly lower than that of the observation group (93.91%; P<0.05).

Conclusion

Letrozole combined with metformin and dydrogesterone has more significant therapeutic effect on PCOS with infertility than letrozole alone. The combination therapy can effectively improve the follicle diameter, correct serum endocrine hormone indicators, and promote the possibility of pregnancy.
