终末期肝泡型包虫病的治疗极为困难和复杂，由于肝泡型包虫病的病灶生长方式早期即可向周围浸润生长，到终末期，病变极易侵及胆道、肝门以及下腔静脉，更有甚者已经通过淋巴道和血管转移到腹膜后和远处器官如脑、肺等部位，常规手术治疗无法切除或达到根治的目的，大多数患者就诊时常常表现为恶病质，因而严重影响患者术后生存期及生活质量。目前主要的治疗方法包括：根治性切除术、肝脏移植术、减体积式病灶切除术、姑息性切除联合药物治疗、药物治疗、射频微波消融术等，其中，手术仍然是目前所有治疗措施中能够为患者提供治愈机会的首选手段。本文根据近年的文献报道，结合我科临床实践，对终末期肝泡型包虫病的治疗现况进行综述。

The treatment of end-stage hepatic alveolar echinococcosis is extremely difficult and complicated. As the lesion of hepatic echinococcosis can infiltrate into the surrounding area in the early stage, the biliary tract, liver, and inferior vena cava are often involved in the end stage, and even worse, metastases to the retroperitoneum and distant organs such as the brain and lung occur via lymphatic and blood vessels. As a result, conventional surgical treatment cannot achieve the goal of curative or radical resection. Most patients often have cachexia at initial diagnosis, which seriously affects their survival period and quality of life after surgery. At present, the main treatment methods include radical resection, liver transplantation, reduced volume resection of lesions, palliative resection combined with drug therapy, drug therapy, radiofrequency, and microwave ablation. Among all the current treatment measures, surgery is still the first choice, which can provide patients with a chance to cure. Based on recent literature reports and clinical practice in our hospital, we review the present situation of treatment of end-stage hepatic vesicular echinococcosis in this paper.
