目的

观察核黄素-紫外线A角膜胶原交联(CXL)术后兔眼角膜基质胶原纤维的变化规律,探讨角膜胶原纤维重塑在CXL术后生物力学稳定性中的作用。

方法

15只新西兰白兔,均设右眼为实验眼,左眼为空白对照眼,经CXL处理后1 d、7 d、4周随机取5只进行观察。角膜行苦味酸天狼星红染色和角膜应力-应变测量,记录CXL术后胶原蛋白成分的变化及最大载荷、最大应力和弹性模量。

结果

正常角膜基质内的胶原纤维排列整齐,与角膜表面平行,以Ⅰ型胶原为主,其间可见少量纤细的Ⅲ型胶原;术后7 d,Ⅲ型胶原纤维明显增多,且纤维明显变粗;术后4周,Ⅲ型胶原纤维显著增多且较粗大,与Ⅰ型胶原间杂分布。与空白对照眼比较,CXL术后1 d,角膜的弹性模量、最大载荷和最大应力增加明显,随着时间流逝,角膜的弹性模量、最大载荷和最大应力有所降低,但与空白对照眼相比仍增加,差异有统计学意义(P<0.05)。

结论

CXL术后角膜各型胶原比例、分布发生改变,提示角膜基质胶原纤维发生重塑,CXL可以增加角膜的生物力学强度,其效应的维持可能与胶原纤维的重塑有关。

Objective

To evaluate the change of corneal collagen after riboflavin-ultraviolet-A induced corneal collagen cross-linking (CXL) on rabbit eyes, and investigate the role of collagen fiber remodeling in biomechanical stability after CXL.

Methods

CXL was performed on the right eye of 15 healthy New Zealand rabbits, and all the left eyes were used as normal controls. Five eyes were randomly chosen at 1 day, 7 days, and 4 weeks post CXL to determine the effect. Picrosirius-polarization and stress-strain tests were performed on corneal specimens to analyze the quantitative change of collagens, peak load, peak stress, and elastic modulus.

Results

The normal corneal collagen fibrils were regularly arranged with a parallel lamellae structure. The predominant collagen type in the normal corneal stroma was collagen Ⅰ, and collagen Ⅲ was dispersed within collagen Ⅰ. By day 7, the amount of type Ⅲ collagen increased and the fiber diameter became thick and asymmetric. By week 4, the amount of type Ⅲ collagen still increased and was intermingled with type Ⅰ collagen in the stroma. During the observed period, the corneal elastic modulus, peak load, and peak stress increased significantly compared with the normal cornea. By day 1, the elastic modulus, peak load, and peak stress reached the highest point and then gradually went down, but still increased significantly than normal controls (P<0.05).

Conclusion

The ratio of type Ⅲ/Ⅰ collagen and their distribution are significantly changed after CXL, which indicates that collagen fiber remodeling occurs and long-term biomechanical stability may be concerned with this remodeling.
