目的

探讨早期梅毒患者血清白细胞介素(IL)-1β、IL-27、IL-33水平与细胞免疫功能指标的相关性。

方法

选择2018年2月至2020年6月河北北方学院附属第一医院收治的73例早期梅毒患者(早期梅毒组)、69例中晚期梅毒患者(中晚期梅毒组)和50例健康志愿者(对照组)。根据快速血浆反应素环状卡片试验(RPR)将早期梅毒组患者分为高RPR滴度组(>1∶4)32例,低RPR滴度组(≤1∶4)41例。检测各组受试者血清IL-1β、IL-27、IL-33水平以及外周血T细胞、NK细胞占比,分析早期梅毒患者血清IL-1β、IL-27、IL-33水平与细胞免疫功能指标的相关性。

结果

早期梅毒组、中晚期梅毒组患者血清IL-1β、IL-33水平高于对照组(P均<0.05),血清IL-27水平低于对照组(P<0.05)。高RPR滴度组患者血清IL-1β、IL-33水平高于低RPR滴度组(P均<0.05),血清IL-27水平低于低RPR滴度组(P<0.05)。早期梅毒组患者治疗后血清IL-1β、IL-33水平下降(P均<0.05),血清IL-27水平上升(P<0.05)。早期梅毒患者血清IL-1β、IL-33水平与CD3+T细胞、CD4+T细胞、CD4+/CD8+、CD3-CD56+NK细胞、CD56brightNK细胞、CD56dimNK细胞占比呈负相关关系(IL-1β:r=-0.819、-0.735、0.773、-0.842、-0.742、-0.793、IL-33:r=-0.802、-0.815、-0.792、-0.863、-0.802、-0.786,P均<0.05),与CD8+T细胞呈正相关关系(r=0.764、0.751,P均<0.05);IL-27与CD3+T细胞、CD4+T细胞、CD4+/CD8+、CD3-CD56+NK细胞、CD56brightNK细胞、CD56dimNK细胞占比呈正相关关系(r=0.737、0.857、0.769、0.812、0.825、0.809,P均<0.05),与CD8+T细胞呈负相关关系(r=-0.738,P<0.05)。

结论

早期梅毒患者血清IL-1β、IL-33水平升高,IL-27水平降低,其可能通过抑制细胞免疫功能参与早期梅毒的发病过程。

Objective

To investigate the correlation of serum interleukin (IL)-1β, IL-27, and IL-33 with cellular immunity in patients with early syphilis.

Methods

Seventy-three syphilis patients with early syphilis, 69 patients with middle-late syphilis (course of infection ≥1 year), and 50 healthy volunteers (control group) were selected at the First Affiliated Hospital of Hebei Northern University from February 2018 to June 2020. According to the rapid plasma reacting ring card test (RPR), the patients in the early syphilis group were divided into either a high RPR titer group (>1∶4) (n=32) or a low RPR titer group (≤1∶4) (n=41). Serum levels of IL-1β, IL-27, and IL-33 and the percentage of T cells and NK cells were determined, and the correlation of serum IL-1β, IL-27, and IL-33 with immune indexes in patients with early syphilis was analyzed.

Results

The levels of IL-1β and IL-33 in the early syphilis group and middle-late syphilis group were significantly higher than those in the control group (P<0.05 each), and the level of IL-27 was significantly lower than that of the control group (P<0.05). The levels of IL-1β and IL-33 in the high RPR titer group were significantly higher than those in the control group and low RPR titer group (P<0.05 each), and the level of IL-27 was significantly lower than that in the low RPR titer group (P<0.05). The levels of serum IL-1β and IL-33 in the syphilis group decreased after treatment (P<0.05 each), while the level of serum IL-27 increased after treatment (P<0.05). Serum IL-1β and IL-33 in early syphilis patients were negatively correlated with CD3+ T cells, CD4+ T cells, CD4+/CD8+, CD3-CD56+ NK cells, CD56brightNK cells, and CD56dimNK cells (IL-1β: r=-0.819, -0.735, 0.773, -0.842, -0.742, and -0.793, respectively; IL-33: r=-0.802, -0.815, -0.792, -0.863, -0.802, and -0.786, respectively; P<0.05 for all), and positively correlated with CD8+ T cells (r=0.764 and 0.751, respectively; P<0.05 for both). Serum IL-27 levels were positively correlated with the levels of CD3+T cells, CD4+ T cells, CD4+/CD8+, CD3-CD56+ NK cells, CD56brightNK cells, and CD56dimNK cells (r=0.737, 0.857, 0.769, 0.812, 0.825, and 0.809; P<0.05 for all), and negatively correlated with CD8+ T cells (r=-0.738, P<0.05).

Conclusion

Serum levels of IL-1β and IL-33 in patients with early syphilis increase, while the levels of IL-27 and IL-1β decrease in such patients. The decrease of IL-27 may participate in the pathogenesis of early syphilis by inhibiting cellular immunity.
