目的

外侧半月板后根撕裂(LMPRT)与前交叉韧带(ACL)撕裂有关,LMPRT可影响膝关节的稳定性。本研究旨在探讨关节镜下ACL+LMPRT一期修复/重建术的中期疗效及功能预后情况。

方法

本研究为回顾性队列研究,研究对象为2017年6月至2020年6月在南通市第一人民医院行镜下重建术治疗的非陈旧性ACL撕裂的45例患者,根据其有无LMPRT,分为LMPRT组(15例)和对照组(30例)。LMPRT组与对照组按照1∶2纳入,且匹配了性别、年龄、体质量指数(BMI)。对2组的术前诊断、关节镜术中录像以及术后复查资料进行回顾和分析。临床指标包括国际膝关节文献委员会(IKDC)和Lysholm膝关节功能评分。

结果

LMPRT组术前Pivot试验阳性率高于对照组(73.33% vs 26.67%,P<0.01)。LMPRT组术后半年的Lysholm评分较对照组偏低(68.47±10.54 vs 74.93±8.11,P<0.05),但术后1年时2组的Lysholm评分比较,差异无统计学意义(P>0.05)。术后半年、术后1年时的IKDC评分在2组之间比较,差异无统计学意义(P>0.05)。

结论

对于非陈旧性损伤而言,ACL+LMPRT一期重建/修复术的手术安全性好、并发症少,术后中期的临床疗效和功能预后良好。建议同期手术时先进行LMPRT修复再进行ACL重建。

Objective

Lateral meniscus posterior root tear (LMPRT) is related to the anterior cruciate ligament (ACL) rupture and can lead to knee instability. This study aimed to investigate the medium-term clinical efficacy and function prognosis of one-staged arthroscopic ACL reconstruction combined with LMPRT repair/reconstruction.

Methods

This is a retrospective cohort study. The research subjects were 45 patients with non-old ACL tears who underwent endoscopic reconstruction at the First People's Hospital of Nantong from June 2017 to June 2020. According to whether they had LMPRT or not, the patients were divided at a ratio of 1∶2 into either an LMPRT group (15 cases) or a control group (30 cases), with gender, age, and body mass index (BMI) matched. The preoperative diagnosis, arthroscopic operation video, and postoperative follow-up data of the two groups were reviewed and analyzed. The clinical indicators assessed included the International Knee Documentation Committee (IKDC) and Lysholm knee function scores.

Results

The LMPRT group had a higher positive rate of Pivot test than the control group (73.33% vs 26.67%, P<0.01). Compared to the control group, the Lysholm score of the LMPRT group was lower at 6 months after surgery (68.47±10.54 vs 74.93±8.11, P<0.05), but there was no statistical significance in the Lysholm score between the two groups at 1 year after surgery (P>0.05). There was no significant difference in IKDC scores between the two groups at 6 months and 1 year after surgery (P>0.05).

Conclusion

One-staged ACL+LMPRT reconstruction/repair for non-old injury has good surgical safety, less complications, and good clinical efficacy and functional prognosis in the mid-postoperative period. It is recommended that LMPRT repair be performed before the ACL reconstruction in one-staged operation.
