目的

探究骨肽注射液联合双氯芬酸钠治疗老年膝关节骨性关节炎(KOA)的效果及安全性。

方法

选择2019年2月至2020年2月首都医科大学附属北京世纪坛医院骨科诊治的老年KOA患者117例作为研究对象,按照治疗方式不同将患者分为对照组(58例)和研究组(59例)。对照组患者给予双氯芬酸钠肠溶片治疗,研究组患者在此基础上联合骨肽注射液,连续治疗14 d。比较2组患者治疗后的效果及不良反应发生率,治疗前后西安大略和麦克马斯特大学骨关节炎量表(WOMAC)、视觉模拟评分(VAS)、Lysholm膝关节评分(LKS)、血清碱性磷酸酶(ALP)及血清骨钙素N端中分子片段(N-MID)水平。

结果

研究组与对照组的治疗总有效率分别为83.05%与68.97%,组间差异具有统计学意义(P=0.002)。治疗前,2组患者血清ALP、N-MID水平差异无统计学意义(P>0.05);治疗后,研究组血清N-MID水平为(18.65±3.79)ng/ml,显著高于对照组[(12.42±2.83)ng/ml,P<0.001];血清ALP水平为(68.43±10.71)U/L,显著高于对照组[(40.24±9.69)U/L,P<0.001]。治疗前,2组患者WOMAC、VAS以及LKS评分差异均无统计学意义(P>0.05);治疗后,研究组WOMAC和VAS评分分别为(18.69±5.25)分和(1.45±0.27)分,显著低于对照组(25.37±6.09)分和(3.16±0.82)分,差异均有统计学意义(P<0.001);治疗后研究组患者LKS评分为(95.07±4.77)分,显著高于对照组(84.62±4.62)分,差异有统计学意义(P<0.001)。在不良反应方面,研究组和对照组分别为5例(8.47%)和11例(18.97%),差异无统计学意义(P=0.099)。

结论

骨肽注射液联合双氯芬酸钠治疗老年KOA的短期疗效显著,安全性较好,有值得进一步研究和临床推广的潜在可能性。

Objective

To evaluate the efficacy and safety of Bone Peptide Injection combined with Sodium Diclofenate in the treatment of knee osteoarthritis (KOA) in the elderly.

Methods

From February 2019 to February 2020, 117 elderly KOA patients at the Department of Orthopedics, Beijing Shijitan Hospital of Capital Medical University were divided into either a study group (59 cases) or a control group (58 cases). The control group was treated with Diclofenac Sodium, while the study group was treated with Bone Peptide Injection combined with Diclofenac Sodium. After treatment, the therapeutic effects and adverse effects were observed, and indexes such as WOMAC, VAS, LKS score, serum ALP, and N-MID levels were compared between the two groups before and after treatment.

Results

The therapeutic response rates of the study group was significantly higher than that of the control group (83.05% vs 68.97%, P=0.002). Before treatment, there were no significant differences between the two groups in serum ALP or N-MID levels (P>0.05); after treatment, the levels of serum N-MID and ALP in the study group were significantly higher than those of the control group [N-MID: (18.65±3.79) ng/ml vs (12.42±2.83) ng/ml, P<0.001; ALP: (68.43±10.71) U/L vs (40.24±9.69) U/L, P<0.001]. There were no significant differences between the study group and control group in terms of WOMAC, VAS, and LKS scores before treatment (P>0.05); after treatment, the scores of WOMAC (18.69±5.25 vs 25.37±6.09, P<0.001) and VAS (1.45±0.27 vs 3.16±0.82, P<0.001) were significantly lower and the LKS score (95.07±4.77 vs 84.62±4.62, P<0.001) was significantly higher in the study group than in the control group. In terms of adverse events, 5 cases (8.47%) were observed in the study group and 11 cases (18.97%) in the control group, there was no significant difference between the two groups (P=0.099).

Conclusion

Bone Peptide Injection combined with Diclofenac Sodium is effective and safe in the treatment of KOA in the elderly, which is worthy of further clinical application.
