    The traditional diagnosis and treatment model for cardiovascular diseases has shown its limitations in the face of a large population of patients with metabolic-related cardiovascular diseases. Based on the 2015 edition, the newly revised Beijing vascular health stratification is the basis for achieving intelligent life-long management of heart and vascular health to improve the prognosis of cardiovascular disease. The traditional model for the diagnosis and treatment of end-stage cardiovascular diseases will be replaced by this new type of health management model, so that the concept of ″preventive treatment of disease″ can be implemented, constructing a comprehensive and standardized service system covering nursing, pension, rehabilitation, diagnosis, and treatment. Thanks to the application of artificial intelligence, internet, and 5G, the largest chronic disease group affecting the population health of China-the burden of vascular-related diseases, which results in the largest chronic disease group in China, will be controlled unprecedentedly, thus contributing to the realization of the dream of healthy China.

    Exploration of psychological support for family members of patients in hospice period

    China has now entered into the aging acceleration period, and the demand for medical care, elderly care, and health services is also greatly increased. In view of the nature of the "combination of medical treatment and care" and the modern medical model of "the combination of biological treatment and psychological treatment" in our hospital, our department has carried out clinical practice on hospice care. Supportive treatment of physical diseases in the elderly is our primary care service, while psychological support for family members is also an important part of our work. The medical staff in our department summarize and develop psychological support services for the dying elderly family members in the long-term clinical work, gradually developing a mature model and drawing the following conclusion: psychological support of hospice patients and their families is very necessary, feasible, and important and can be very effective to improve our service levels and clinician-patient communication. At the same time, hospice care services need the participation of doctors of many disciplines such as medicine, sociology, law, ethics, psychology, and pedagogy to promote the development of hospice care.

    Effect of Combination of Medical-Health Care-Psychological pension model

    Objective

    To explore a suitable service model of institutional pension by analyzing the validity of the Combination of Medical-Health Care-Psychological pension model that was developed and practiced for a long term by the clinical psychiatric team in Kunming Social Welfare Institute Welfare Hospital.

    Methods

    80 elderly inpatients were divided into a study group and a control group. The elderly in the study group received the Combination of Medical-Health Care-Psychological pension model service, and the control group received the general medical care mode service. The quality of life, wellbeing, and satisfaction were assessed by interview and with a health status questionnaire, the Memorial University of Newfoundland Scale of Happiness (MUNSH), and life satisfaction scale. t-test was used to compare the difference of indexes between groups.

    Results

    Integrating supportive group psychological services in the pension model could enhance the positive psychological experience, deal with negative psychological experience, and improve the quality of life and satisfaction (P<0.05). The integrated model could improve the health status in the elderly by promoting the affective function, social function, physical pain, psychical energy, and mental health (P<0.05).

    Conclusion

    The Combination of Medical-Health Care-Psychological pension model is very suitable for the elderly in the institutions and has a very positive role in promoting their physical and mental health.
