目的

分析光动力疗法(PDT)联合薇诺娜屏障特护霜治疗光化角化病(AK)的疗效及对人类RUNT相关转录因子3(RUNX3)、B淋巴细胞瘤-2(Bcl-2)、神经酰胺(Cer)水平的影响。

方法

选取四川大学华西医院2016年1月至2021年6月期间收治的AK患者116例作为研究对象,按照随机数字表法分为对照组与研究组,各组58例。对照组给予PDT治疗,研究组给予PDT和薇诺娜屏障特护霜治疗。比较2组临床疗效、RUNX3、Bcl-2、Cer水平及炎症因子水平、不良反应、复发率、生活质量(DLQI)。

结果

研究组完全缓解率(79.31%)高于对照组(62.07%)(P<0.05);研究组不良反应发生率、复发率及DLQI评分低于对照组(P<0.05),2组治疗总有效率差异无统计学意义(P>0.05);研究组治疗后TNF-α、IL-6、ICAM-1低于对照组,Cer高于对照组(P<0.05);2组治疗后RUNX3、Bcl-2阳性率相比,差异无统计学意义(P>0.05)。

结论

PDT联合薇诺娜特护霜治疗AK疗效显著,明显降低治疗不良反应发生,皮肤修复迅速,减少复发,促进Cer合成,提高生活质量。

Objective

To evaluate the efficacy of photodynamic therapy (PDT) combined with Winona barrier extreme cream in the treatment of actinic keratosis (SK) and its effects on the levels of human runt related transcription factor 3 (RUNX3), B lymphoma-2 (Bcl-2), and ceramide (CER).

Methods

One hundred and sixteen patients with AK treated at West China Hospital of Sichuan University from January 2016 to June 2021 were randomly divided into a control group and a study group, with 58 cases in each group. The control group was treated with PDT, and the study group was treated with photodynamic therapy combined with Winona barrier extreme cream. The clinical efficacy, RUNX3, Bcl-2, CER, inflammatory factors, adverse reactions, recurrence rate, and quality of life were compared between the two groups.

Results

The complete remission rate in the study group (79.31%) was higher than that of the control group (62.07%; P<0.05). There was no significant difference in the total effective rate, rate of adverse reactions, or recurrence rate between the two groups (P>0.05). After treatment, TNF-α, IL-6, and ICAM-1 levels in the study group were lower than those in the control group, and Cer was higher than that in the control group (P<0.05). There was no significant difference in the positive rates of RUNX3 and Bcl-2 between the two groups after treatment (P>0.05). The DLQI score of the study group was lower than that of the control group (P<0.05).

Conclusion

PDT combined with Winona barrier extreme cream is effective in the treatment of AK. The combination therapy has anti-inflammatory effects, promotes ceramide synthesis, improves the quality of life, and is associated with a low rate of adverse reactions and recurrence.
