目的

探讨在中老年人群中载脂蛋白B(ApoB)水平与冠状动脉狭窄严重程度(Gensini评分)之间的关系。

方法

本研究为横断面回顾性研究,纳入2022年1月至6月在长治市人民医院接受冠状动脉造影检查的45岁以上患者818例。根据Gensini评分将患者分为轻度狭窄组(Gensini评分<36)和中重度狭窄组(Gensini评分≥36)。采用二元Logistic回归分析ApoB水平与冠脉狭窄严重程度的相关性,并调整年龄、性别、BMI、高血压、糖尿病、吸烟史等混杂因素。

结果

818例患者中,轻度狭窄组547例,中重度狭窄组271例。多因素Logistic回归分析显示,ApoB水平与中重度冠脉狭窄呈显著正相关(OR=1.24,95%CI:1.15~1.34,P<0.001)。在校正混杂因素后,ApoB每升高1 g/L,中重度冠脉狭窄的风险增加24%。

结论

本研究揭示了ApoB水平与中老年人群冠状动脉狭窄严重程度之间存在显著的正相关关系。该发现可能提示ApoB可作为评估冠脉粥样硬化负荷的补充指标,但本研究结论仅限于相关性层面,不能推导因果关系。

Objective

To explore the association between apolipoprotein B (ApoB) levels and the severity of coronary artery stenosis, as measured by the Gensini score, in middle-aged and elderly populations.

Methods

This cross-sectional retrospective study included 818 patients aged 45 years and older who underwent coronary angiography at Changzhi People's Hospital between January and June 2022. Based on their Gensini scores, patients were classified into a mild stenosis group (Gensini score <36) and a moderate-to-severe stenosis group (Gensini score ≥36). Binary logistic regression analysis was used to assess the association between ApoB levels and the severity of coronary stenosis, adjusting for confounding factors such as age, sex, body mass index, hypertension, diabetes, and smoking history.

Results

Of the 818 patients included, 547 were assigned to the mild stenosis group and 271 to the moderate-to-severe stenosis group. Multivariate logistic regression analysis revealed a significant positive correlation between ApoB levels and moderate-to-severe coronary stenosis (OR=1.24, 95%CI: 1.15~1.34, P<0.001). After adjusting for confounding factors, each 1 g/L increase in ApoB was associated with a 24% increase in the risk of moderate-to-severe coronary stenosis.

Conclusion

This study demonstrates a significant positive correlation between ApoB levels and the severity of coronary artery stenosis (Gensini score) in middle-aged and elderly populations. These findings suggest that ApoB may serve as a supplementary marker for assessing the burden of coronary atherosclerosis. However, the conclusions of this study are limited to demonstrating a correlation and do not imply a causal relationship.
