Effect of hospital-community integration pattern on management of hypertension

Abstract:

Objective

To investigate the effect of the hospital-community integration pattern on the management of hypertension.

Methods

We randomly chosen 10 communities, which had community-based health service centers, in Liupanshui, Guizhou Province. Then, we randomly divided these communities into 2 groups, with each group containing 5 communities. One group adopted the hospital-community integration pattern for the management of hypertension (hospital-community pattern), and the other group used the hospital follow-up pattern (hospital pattern). A total of 1005 patients were included, of which 502 were in the hospital-community pattern group and 503 in the hospital pattern group. All patients were followed at 1, 3, and 6 months after inclusion. The differences of medical compliance and blood pressure were evaluated between the two groups. Logistic regression analysis was performed to investigate the independent predictors of uncontrolled blood pressure.

Results

During the follow-up period, compliance was increased gradually in both groups (P<0.05). The compliance significantly rose from 73% to 91.8% in the hospital pattern group, and from 76.1% to 92.4% in the hospital-community pattern group (P<0.001); however, the difference of compliance between the groups was insignificant at each follow-up time point (P>0.05). At 6 months, systolic blood pressure was lower in the hospital-community pattern group than in the hospital pattern group (P<0.05). Result from logistic regression analysis showed that hospital pattern was an independent predictor of uncontrolled blood pressure. Compared with the hospital-community pattern, hospital pattern had was associated with a 1.99-fold increased odds of uncontrolled blood pressure (P<0.05).

Conclusion

The hospital-community integration pattern can significantly improve the compliance and blood pressure control rate in patients with hypertension, which deserves to be promoted in the prevention and treatment of hypertension.
