Multivariate analysis of risk factors for positive surgical margins following conization for cervical intraepithelial neoplasia grade 3

Abstract:

Objective

To identify the risk factors for positive surgical margins in patients with cervical intraepithelial neoplasia (CIN) grade 3 undergoing cold knife conization.

Methods

This study included patients with CIN grade 3 who underwent cold knife conization between October 2010 and June 2016. The risk factors related to the positive surgical margins were analyzed by multivariate regression analysis.

Results

Conization was performed in 407 patients, of whom 74 had positive surgical margins and 333 had negative surgical margins. Factors associated with positive surgical margins included gravidity (OR = 1.202, 95% CI: 1.025-1.409), HPV infection (OR = 2.231, 95% CI: 1.177-4.230), and number of lesions in the cervix (OR = 1.163, 95% CI: 1.086-1.247).

Conclusion

HPV, gravidity, and number of lesions in the cervix are associated with positive surgical margins after conization for CIN grade 3.
