Premature death of residents in a suburb community of Shanghai in process of urbanization

Abstract:

Objective

To investigate the situation and trend of premature death of residents in a suburban community of Shanghai in the process of urbanization.

Methods

According to the mortality data of residents from 2009 to 2018 in Wanggang community, potential years of life lost (PYLL), age-standardized PYLL, average years of life lost (AYLL), and annual percent change (APC) were calculated to analyze the status and trends of those rates with a join-point regression model.

Results

From 2009 to 2018, 2437 residents died, of which 629 were under 70 years old, accounting for 25.81%(629/2437). The total PYLL was 8099.20 person-years, the PYLL rate was 23.18‰, the standardized PYLL rate was 14.05‰, and the AYLL rate was 3.32 year-person. PYLL showed a significant downward trend year by year [APC (95%CI): -3.98 (-7.29, -0.54)%, P=0.03], and AYLL also showed a significant downward trend year by year [APC (95%CI): -5.72 (-9.44, -1.84)%, P=0.01]. The main causes of premature death were malignant tumors, traffic accidents, and circulatory diseases. The PYLL rates were 11.27‰, 2.76‰, and 2.07‰, respectively. The main AYLL losses were traffic accidents, suicide, and malignant tumors. AYLL were 13.37‰, 12.75‰, and 5.03‰, respectively.

Conclusion

The premature death of local residents shows a downward trend. Prevention of traffic accidents and suicide is an effective way to control premature death.
