    Health conditions and strategies for care of rural elderly people in Jiangsu Province from perspective of active aging

    Objective

    To analyze the living and health conditions of rural elderly people in Jiangsu Province and discuss measures that can be taken to improve and implement the elderly care policy in rural areas from the perspective of positive aging.

    Methods

    A multi-stage stratified sampling survey was conducted on the living and health conditions of rural elderly people in Jiangsu Province using a self-written questionnaire, and descriptive analysis was performed to analyze the data.

    Results

    The top three prevalent chronic diseases among the elderly in rural areas were hypertension, osteoarthritis, and cardiovascular and cerebrovascular diseases. The prevalence of chronic diseases was 67.93%, and the prevalence of two or more chronic diseases was 39.67%. The degree of project completion was fair. Of all the elderly people surveyed, 43.84% can provide living support to their childre, 37.68% can provide financial support to their children, 54.35% did not live with their children, 15.58% had no pension insurance, and 12.5% needed life care, with the main caregivers being spouses (49.28%) and children (44.93%).

    Conclusion

    The health conditions of the elderly in rural areas is poor, we should strengthen the health management of the elderly should be strengthened and their health needs should be met. The rural elderly have a high degree of participation in family activities, and necessary social support should be provided for them. The level of old-age pension security is not high, and rural old-age pension security policies in line with national conditions should be formulated.

    Association of selenium and selenoprotein with blood lipids in elderly people

    Objective

    To investigate the association between the levels of selenium and blood lipids in middle-aged and elderly people, and assess the mediatory effect of superoxide dismutase (SOD) in their association.

    Methods

    A cross-sectional study was conducted in 85 middle-aged and elderly people in Yichun. Blood samples were collected to determine the concentrations of selenium, selenoproteins [selenoprotein P (SEPP1) and glutathione peroxidase (GPX)], and blood lipids. Linear regression analyses were performed to investigate the association between the levels of selenium and SOD, as well as the association between the levels of selenium and blood lipids. Then,selenium biomarkers related to blood lipids were identified to further investigate whether there is any mediatory effect in the relationship between selenium biomakers and blood lipids.

    Results

    Linear regression analysis revealed that the level of selenium in plasma was not correlated with blood lipids, but the selenoproteins (SEPP1 and GPX) were negatively associated with low density protein cholesterol (LDL-C) and total cholesterol (TC). In addition, selenoproteins were found to be associated with SOD (P<0.05). In the mediation analysis, SOD was found to be a significant mediator of the association between SEPP1 and LDL-C (P<0.05).

    Conclusion

    Selenoproteins may reduce blood lipid levels by inhibiting oxidative stress.

    Health status of the elderly in a community in Chengdu and influencing factors

    Objective

    To identify the factors influening the health status and common health problems among the elderly in a Chengdu community, in order to provide a theoretical basis for community health care for the elderly.

    Methods

    Elderly people aged ≥ 65 years old who underwent physical examination at Jinyang Community Health Service Center, Chengdu in 2020 were selected as the research subjects. They were divided into groups according to sex and age. Chi-square test was used to analyze whether there were differences in body mass index, blood pressure, blood glucose, blood lipid, anemia, abnormal liver function, and abnormal renal function. Logistic regression was used to analyze the influencing factors of hypertension, diabetes, and impaired fasting blood glucose.

    Results

    A total of 5589 elderly people were included, in which common health problems (in a descending order) were hypertension, dyslipidemia, overweight or obesity, diabetes, abnormal liver function, abnormal renal function, impaired fasting blood glucose, and anemia. The detection rate of obesity and dyslipidemia in women was higher than that in men, and the detection rate of impaired fasting blood glucose and abnormal renal function in men was significantly higher than that in women (P<0.05). The detection rates of dyslipidemia, overweight, abnormal liver function, diabetes, hypertension, anemia, and renal dysfunction were significantly different across different age groups (P<0.05); the detection rate of hypertension, anemia, and renal dysfunction was the lowest in the age group of 65~74 years old, and the highest in the age group ≥85 years old, and showed an obvious increasing trend with the increase of age. The prevalence rate of comorbid chronic diseases was significantly different between different genders, ages, and body mass indexes (P<0.05). The prevalence rate of comorbidities in women was higher than that in men, and it was the highest in the ≥85 years old group. The prevalence rate of comorbidities in the obesity group was the highest. Multivariate Logistic regression analysis showed that age, overweight, obesity, elevated triglyceride, abnormal liver function, and abnormal renal function were the influencing factors of hypertension; elevated triglyceride and abnormal liver function were the influencing factors of diabetes; and overweight, obesity, elevated low-density lipoprotein, elevated triglyceride, and abnormal liver function were the influencing factors of impaired fasting blood glucose.

    Conclusion

    The health status of the elderly in the community is not optimistic. The health problems of elderly people of different genders and ages are different. The risk factors for hypertension, impaired fasting blood glucose, and diabetes are different, and the prevalence rate of "hypertension, hyperlipidemia, and hyperglycemia" is high. Therefore, individualized health management should be carried out for the elderly to improve their health level.

    Application of "specialty-general practice" integrated management in elderly patients with non-valvular atrial fibrillation and heart failure: a propensity score matching study

    Objective

    To evaluate the efficacy of the "specialty-general practice" integrated management model in patients with non-valvular atrial fibrillation (NVAF) complicated with non-heart failure with preserved left ventricular ejection fraction (HFpEF).

    Methods

    A total of 184 patients who were diagnosed as having NVAF with non-HFpEF at the Suzhou Ninth Hospital Affiliated to Soochow University from December 2019 to December 2021 were retrospectively enrolled in the study. The patients were divided into either a standardized group consisting of heart failure patients (141 cases) managed using the traditional mode of diagnosis and treatment and follow-up of cardiology department, or an integrated management group consisting of heart failure patients (43 cases) who were managed using the traditional mode of cardiology specialist combined with the community general practitioner team. Gender, age, smoking, type 2 diabetes mellitus, hyperlipidemia, hypertension, coronary heart disease, chronic obstructive pulmonary disease, LVEF, and standardized drugs (anticoagulants, ARNI/ACEI/ARB, beta-blockers, diuretics, spironolactone, digoxin, etc.) were matched with a 1∶1 propensity score, and then the therapeutic effects of the two groups were compared.

    Results

    Propensity score matching was successful for 41 pairs of cases. After propensity score matching, compared with the standardized group, the integrated management group had significant improvements in 6-minute walk test (6MWT), NT-proBNP, irregular drug withdrawal, Minnesota Heart Insufficiency Quality of Life Scale (MLHFQ) score, multiple heart failure readmission rate, and 1-year major adverse cardiovascular events (MACE) (P<0.05). There was no significant difference in ventricular rate, LVEF value, or device therapy (ICD, CRT, CRT-D, catheter radiofrequency ablation, and left atrial appendage closure) between the two groups (P>0.05).

    Conclusion

    Cardiovascular specialists combined with community general practitioner team to carry out integrated joint management of elderly patients with NVAF complicated with non-HFpEF can significantly reduce the number of heart failure hospitalizations and the occurrence of MACE events while improving patient compliance and quality of life.

    Application of the Manual on Contracted Family Doctor Services for Persons with Disabilities in physically disabled persons: a qualitative study

    Objective

    In order to improve the quality of contracted family doctor services for persons with disabilities, the Department of Primary Health, the National Health Commission of the People's Republic of China, and the Rehabilitation Department of the China Disabled Persons' Federation jointly issued the Manual on Contracted Family Doctor Services for Persons with Disabilities (hereinafter referred to as the Manual) in August 2018. By investigating the physical disabled persons with chronic diseases, this sudy aimed to find out the problems in the implementation of the Manual and provide a basis for further modification of the Manual.

    Methods

    Semi-structured interviews were conducted from August to December 2019, Twelve physically disabled persons with chronic diseases in Yuetan Community Health Service Center, Beijing's Xicheng District were selected by convenient sampling method.

    Results

    According to the level of disability, 66.7% (8/12) of the interviewees signed the contract in community health centers, and 33.3% (4/12) of the interviewees signed the contract in their home. The rates of chronic disease follow-up, health education, and physical examination were all 100%. Approximately 41.7% (5/12) of the interviewees had received treatment of concomitant diseases and complications. The demand rate of rehabilitation treatment was 100%, the satisfaction rate was 83.3% (10/12), and 16.7% (2/12) of the interviewees wanted to be referred to the rehabilitation hospital. About 75.0% (8/12) of the interviewees had door-to-door service demand, and the actual satisfaction rate was 37.5% (3/8). All the interviewees and their family members expressed that they had a need for nursing knowledge, and 25.0% (3/12) of the interviewees had received nursing guidance from community nurses. All interviewees expressed that they had little contact with the full-time members of the disabled committee, and 4/12 (33.3%) interviewees proposed that rehabilitation equipment was obsolete and needed to be updated.

    Conclusion

    The implementation of the Manual has promoted the communication between family doctors and physically disabled persons, and has played an appreciated role in improving the quality of contracted services of family doctors for the disabled. Limited by the insufficient number of family medical team members and the need to improve their competence, some services such as door-to-door service, rehabilitation treatment, and referral can not be fully achieved. Relevant contents such as training for staff and physically disabled, establishment of rehabilitation medical alliances, and standardized management of door-to-door services can be added to the Manual.

    Informatization construction of public health in primary medical institutions in Jiangsu Province

    Objective

    To explore the informatization construction of public health in primary medical institutions and to provide countermeasures.

    Methods

    From July to August 2021, an electronic questionnaire survey was conducted in 246 primary medical and health institutions in Jiangsu Province. The status and the existing problems of basic public health service informatization including residents' health records, key population health management, and traditional Chinese medicine health management were ananlyzed.

    Results

    The informatization construction in electronic health records (EHR) of the residents, chronic disease management and key population health management in the surveyed institutions were relatively good, but the EHR sharing rate and the utilization rate were relatively low. Out of all the surveyed institutions, 32.5% (80/246) opened the EHR information inquiry service out of the hospital, 47.2% (116/246) integrated the EHR into the hospital information system, and 52.4% (129/246) integrated the electronic medical records into the health record system. The informatization level of chronic disease and key population health management in the surveyed institutions was slightly higher than the the national level, but the information sharing rate was poor, and the rate of integrating with provincial (national) platforms was only 50.0%-67.9%. Approximately 53.3% (131/246) of the surveyed institutions constructed the informatization of traditional Chinese medicine health management, and 44.3% (109/246) were very satisfied with the public health informatization.

    Conclusion

    The overall development of the informatization of public health services in primary medical institutions is good, but there are still shortcomings and deficiencies. The top-level design strategy should be improved, the informatization process of grassroots Chinese medicine and other projects should be promoted, and the integration and sharing of medical and preventive data should be promoted, so as to improve the informatization construction in an all-round way, which is important for building a happy and healthy China.

    Objective

    To investigate the effect of community group psychological intervention on the mental health and sleep health of the elderly, to provide a basis for improving the psychological and sleep health of elderly people.

    Methods

    A community group psychological intervention was carried out in a community of Beijin The psychological and sleep health of elderly people in the Kaiyangli community of Fengtai District, Beijing before and after community intervention was investigated from October 15, 2018 to January 2, 2019 using the self-rating anxiety scale (SAS), self-rating depression scale (SDS), and pittsburgh sleep questionnaire (PSQI). A total of 64 questionnaires were administered, and 58 were valid. The intervention was group psychological activities carried out by professional psychologists for the elderly in the community, once a week, for a total of 8 weeks, and each activity lasted 90 minutes.

    Results

    Before the intervention, the average SAS score of the elderly in the community was (47.10±1.03), and after group psychological intervention, the score was (43.60±1.03); there was a significant difference between before and after the intervention (t=3.358, P=0.001). The average SDS score [(53.16±1.47) vs (50.07±1.35), t=2.056, P=0.044] and average PSQI score [(6.16±0.52) vs (4.61±0.49), t=4.360, P<0.001] were also significantly lower after the intervention than before the intervention.

    Conclusion

    Group psychological intervention can not only improve the anxiety and depression of elderly people in communities, but also improve their sleep status, which is worthy of adoption and promotion.

    After the outbreak of Corona Virus Disease 2019 (COVID-19), relevant enterprises and institutions have adopted the "emergency braking " method for epidemic prevention and control, which played a positive role in the control of the epidemic. At present, enterprises and construction sites across the country have returned to work and production in an orderly manner. Under the unified leadership of the Chinese government, medical and health institutions have actively assisted in the prevention and control of the epidemic in construction site and enterprises, ensuring that both the epidemic prevention and economic activities are not delayed. This article summarizes the epidemiological knowledge of the novel coronavirus and COVID-19 based on the literature, national laws, regulations, and regulatory documents. At the same time, we propose hygiene measures and employee self-health management recommendations applicable to construction site and enterprises to provide reference for general practitioners and prevention and control personnel.

    Medical service status of pediatric outpatient and emergency department of a tertiary hospital in Beijing during COVID-19 epidemic: implications for the future

    Objective

    To investigate the impact of COVID-19 epidemic on pediatric outpatient and emergency department and the possible causes, to provide reference for optimizing children′s medical-seeking pattern and achieving children′s hierarchical diagnosis and treatment.

    Methods

    A self-designed questionnaire was used to investigate the children who visited the pediatric outpatient or emergency department of Peking University First Hospital from 08:00 on February 25, 2020 to 08:00 on the next day, as well as their accompanying relatives and corresponding doctors. A semi-structured interview method was used to collect data from pediatricians responsible for the outpatient and emergency department during the COVID-19 epidemic.

    Results

    A total of 77 valid questionnaires were returned with a response rate of 87.5%, which showed that almost half of the children (51.9%, 40/77) were absent and their parents described the main complaints. Forty-four (57.2%, 44/77) visits resulted from consulting/prescription, and the other 33 (42.8%, 33/77) were led by various complaints, among which the five leading complaints were fever, cough, rash, jaundice, and premature baby follow-up, the five leading diseases were pneumonia, suppurative tonsillitis, upper respiratory infection, neonatal jaundice, and the follow-up of premature infants. Of these 33 visits, 13 (39.4%) cases were evaluated by their corresponding doctors as being able to get diagnosed and treated at a qualified primary medical institution instead of coming here, and 21 (63.6%) and 5 (15.2%) cases were considered to be able to get some and all problems solved through online consultation, respectively. The qualitative interview results showed that since the outbreak of COVID-19, the number of children visiting outpatient or emergency department had reduced significantly. Compared with the situation before the epidemic, the conditions of children visiting emergency department became more complicated, while those of children visiting outpatient department became simpler. Meanwhile, the doctor-patient relationship became more harmonious. In the future, multiple measures should be taken to promote the triage of outpatient and emergency patients visiting pediatrics department of tertiary hospitals or Children′s Hospital.

    Conclusion

    The epidemic of COVID-19 has a huge impact on the health-seeking behavior of children and their parents in the outpatient and emergency department of general hospitals. In the future, we should strength medical science education for parents, develop online medical consultation, improve the routes of drug availability for chronic disease patients, strengthen doctor training, and develop medical conjoined body, so as to change the children′s medical care behavior and promote the implementation of hierarchical medical system in children.

    Since the outbreak of the Corona Virus Disease 2019 (COVID-19), people′s life and health have been greatly threatened. Both mothers and newborn babies are susceptible groups, and it is important to ensure their safety. After two months of efforts, the epidemic situation has improved dramatically. As a result, the number of outpatients has increased significantly, and infection prevention and control in neonatal clinics, including medical procedures, protective details, identifying high-risk neonates, etc., have become new challenges to medical staff. Based on the guidelines and expert consensus issued by various academic institutions in China, combined with the actual clinical work experience of our hospital, this article provide ideas for infection prevention and control in neonatal clinics under the improved epidemic situation to ensure the health of newborn babies and the safety of medical staff.

    Since December 2019, COVID-2019 has emerged in China. With the development of the epidemic, it is now believed that people of all age groups are generally susceptible to COVID-2019. Reported children cases often had mild symptoms and most of them occurred within the family. The pediatrics department of general hospitals has undertaken outpatient and emergency medical work for nearly half of children. Under the current epidemic situation, the task should be focused on prevention and control screening. Based on the first-line clinical diagnosis and treatment experience and the characteristics of confirmed children cases, this paper discusses the prevention and control strategy and the suggestions for pediatric grading control to strengthen the management of asymptomatic children or those with mild symptoms and prevent them becoming a source of infection in the community and even in medical institutions.

    In December 2019, a COVID-19 outbreak occurred in Wuhan, Hubei Province and quickly spread to other regions of China. With the formulation of policies, implementation of measures, and public participation, the epidemic situation in China has been effectively contained. However, the epidemic situation in other countries is becoming more and more serious. This article takes the Health Commission of Xiangcheng District as an example to elaborate the specific measures to prevent and control the COVID-19, and puts forward some suggestions for the rural health administrative departments of China and even other countries, with an aim to provide some reference and guidance to prevent and control COVID-19.

    Objective

    Summarize the healthcare management and related experiences of the 2nd Foshan Medical Team in the rescue of coronavirus disease 2019 (COVID-19).

    Methods

    The human resource, baseline information, reported healthcare events including uncomfortable symptoms, onset sites, duration, management and prognosis were analyzed retrospectively.

    Results

    There were 160 medical staffs joined the 2nd Foshan Medical Team. And 104 healthcare events reported by 33 medical staffs were issued. The major reported symptoms are 56.7% with cough and 13.5% with dizziness / headache. 76.9% reported symptoms lasted one day. Most reported healthcare events happened at hotel and 90.38% released by having rest. Only 6.7% had got medicine and 5.8% were scanned by CT and got nucleic acid amplification testing with negative results.

    Conclusion

    The successful healthcare management of medical staff is the cornerstone for the whole medical team to complete the mission and return home peacefully. The 2nd Foshan Medical Team set up a staff healthcare coordinator to monitor all staffs′ health situation, submit and solve the healthcare event, and set up an algorithm for healthcare and urgent rescue, and enhance medical staffs′ psychological care. Hence, all medical staffs are health and return home peacefully. The related experiences of the 2nd Foshan Medical Team is useful.

    Objective

    To explore survey the status of anxiety and depression in residents of Nanning during the epidemic of Corona Virus Disease 2019 (COVID-19) to provide reference for psychological counseling of home residents in this special period.

    Methods

    From February 7 to February 14, 2020, a self-made general information questionnaire, the serf-rating anxiety scale (SAS), and the serf-rating depression scale (SDS) were administered via WeChat platform to investigate the anxiety and depression status of 312 residents in Nanning.

    Results

    The mean SAS score in this study was (39.44±9.77), which was significantly higher than the Chinese norm (37.23±12.59, t=4.00, P<0.01). The mean SDS score was (45.22±11.74), which was also significantly higher than the Chinese norm (41.88±10.57, t=5.03, P<0.01). Multiple linear regression analysis showed that worrying about being infected and sleep disorder were risk factors for anxiety; low education level and sleep were risk factors for depression.

    Conclusion

    During the COVID-19 epidemic, residents of Nanning have negative feelings of anxiety and depression, especially those who are overly worried being about infected people, those with poor sleep quality, and those with a lower education level. We should pay more attention to the residents mental and psychological health.

    Corona Virus Disease 2019 was confirmed in January 2020 in Wuhan, and the Chinese government has adopted a series of prevention and control strategies across the country to fight against the epidemic, effectively containing the rapid spread of the epidemic. The prevention and health care departments of community hospitals, as the most grass-roots organizations for disease prevention and control, responded quickly, and set up different groups such as the sampling and testing result feedback group, the epidemiological investigation group, the home isolation management group, the external disinfection and sterilization group, the information collection and submission group, and the emergency material management group, as well as the centralized observation management and health communication group, each of which had its own duty, effectively building up the community first-line defense and control fortress.

    The national epidemic caused by COVID-19 is still ongoing, the number of confirmed cases continues to increase, and the prevention and control of the epidemic are in a grim moment. Primary medical institutions are the basis of grid management and the first line of defense for prevention and control of infectious diseases. We should earnestly implement comprehensive prevention and control measures at the grass-roots level, and strengthen the pre-examination, triage, and management of fever patients to achieve the goals of "early detection, early report, early isolation, early diagnosis, and early treatment", which will effectively curb the spread of the epidemic. At present, there are few reports about COVID-19 community prevention and control. This paper gives a detailed description of the practice of fever patient management in urban community health service centers during the COVID-19 epidemic, with an aim to provide suggestions for the prevention and control of the epidemic at the primary medical institutions.

Taking the professional advantages of primary community health services in community prevention and control is important to effectively contain the epidemic, protect the healthy population, and defeat the epidemic. Based on the relevant release of the National Health Commission and the Chinese Center for Disease Control and Prevention, this paper aimed to explore the measures and methods taken for the prevention and control of the epidemic within communities to provide ideas for response to major public health emergencies.

    Taking the Fourth People′s Hospital of Wujiang District of Suzhou as the research object, the advantages and disadvantages of family doctor team in community grid prevention and control are analyzed in this paper. We believe that family doctor team plays an irreplaceable role in the grid prevention and control of the COVID-19 epidemic community, but the comprehensive ability of family doctor team still needs to be improved, which will be beneficial to the continuous improvement of the emergency safeguard system for public health emergencies.

    At the end of 2019, a novel coronavirus appeared in China for the first time, resulting in a national epidemic. Since the discovery of this virus, with the increase of confirmed cases and the continuous updating of scientific research data, we found that hypertensive patients with Novel Coronavirus Pneumonia 2019 have different disease outcomes compared with ordinary patients. Therefore, there is a greater need for scientific prevention and management of the infection among hypertensive people. This paper puts forward some issues that should be paid attention to in community management of hypertension and gives related practical management measures.
