    Clinicopathologic characteristics of inflammatory breast cancer cases: analysis of 25 cases

    Objective

    To investigate the clinicopathologic features and prognostic factors of inflammatory breast cancer (IBC).

    Methods

    We retrospectively analyzed the clinicopathologic characteristics and outcomes of patients with IBC diagnosed at Peking University First Hospital from January 2012 to December 2018.

    Results

    A total of 25 patients with IBC were included, accounting for 0.89% (25/2811) of newly diagnosed breast cancer cases at our center. All patients had complete clinicopathologic data and follow-up results. Seventeen (68%) patients had stage Ⅲ IBC, and eight (32%) had stage Ⅳ disease. The numbers of patients with luminal B (HER-2 negative), HER-2 positive, and triple-negative breast cancer were 9 (36%), 5 (20%), and 11 (44%), respectively. Nineteen patients underwent surgery, 94.7% of whom achieved local R0 resection. The median survival time was 18.2 months. The 2-year overall survival rates of patients with prognostic stage ⅢB, stage ⅢC, and stage Ⅳ disease were 72.9%, 66.7%, and 15.6%, respectively; there was a significant difference among them (χ2=13.247, P<0.001).

    Conclusion

    IBC is a rare, aggressive disease with advanced stage and poor disease outcome. Local R0 resection can be achieved in more than 90% of patients after neoadjuvant therapy.

    Tumorigenesis and tumor progression are inseparable from the tumor ecosystem consisting of the tumor itself and its microenvironment. The clinical translation from bench to bedside can help breast oncologists develop more comprehensive and effective treatment strategies for breast cancer. The rise of immunotherapy is changing the pattern of systemic treatment of breast cancer. Combination of immunotherapy with traditional treatment can further improve the prognosis of breast cancer patients. In the future, discovering new targets and more accurate predictive biomarkers for anti-tumor immunotherapy are of importance to improve the clinical effect of immunotherapy for breast cancer patients.

    Chinese Society of Breast Surgery (CSBrS) has been focusing on promoting the development and standardization of diagnosis and treatment of breast cancer in China since its establishment. During 2017 and 2021, CSBrS published 11 consensuses on fundamental issues of breast surgery and carried out 23 multi-center real world studies. Clinical practice guidelines for breast surgery (2021), using GRADE system based recommendations, conducted by CSBrS will be promulgated soon. It is hoped that these guidelines will lay equal stress on clinical research and guidelines, and promote the development of breast surgery in China.

    Deep-learning computer-assisted decision-making (DL-CAD) medical device software has high technical content, high clinical value and high social attention. Center for Medical Device Evaluation (CMDE) has introduced several policies to encourage technological innovation and promote industry development. As an innovative clinical application technology, neither manufacturer nor reviewer has enough experience with registration and evaluation of DL-CAD devices. In consequence, many misunderstanding exist in its clinical evaluation requirement. This article aims to provide reference for investigators, manufacturers and reviewers, as well as summarize elements of clinical trial design, and bias control methods, by investigating and surveying global supervision requirements & experience, academic research achievements.

    Phenotypic characteristics and drug resistance of Klebsiella pneumoniae

    Objective

    To analyze the different biological phenotypic characteristics of Klebsiella pneumoniae and provide an experimental basis for clinical anti-infection treatment.

    Methods

    Clinical Klebsiella pneumoniae isolates were collected from the Qingdao Municipal Hospital, China from January 2018 to October 2019. VITEK-2 compact automatic bacterial identification/susceptibility sensitivity system was used for identification and drug sensitivity testing of clinically isolated strains. The origin and distribution of Klebsiella pneumoniae specimens were analyzed, and the differences in drug resistance rates of Klebsiella pneumoniae with different biological phenotypes were compared.

    Results

    A total of 1435 strains of Klebsiella pneumoniae were isolated, among which 316 (22.0%) produced extended-spectrum beta-lactamases (ESBLs), 177 (12.3%) were carbapenem-resistant (CR-KP), and 113 (7.9%) were high-virulent (HvKP). Sputum specimens were the most abundant source, in which 489 strains were detected, accounting for 34.1%; the second was urine specimens, with 220 strains isolated, accounting for 15.3%. The departments involved were mainly intensive care unit, emergency ward, and neurosugical ward, accounting for 21.8%, 15.4%, and 10.8%, respectively. Drug sensitivity results showed that the sensitivity of Klebsiella pneumoniae with different biological phenotypes to commonly used antibiotics was quite different; CR-KP had the highest resistance, with a drug resistance rate to most antibiotics being above 80%, followed by ESBLs, and HvKP strains had the lowest resistance, with a drug resistance rate to most antibiotics being less than 10%.

    Conclusion

    Klebsiella pneumoniae causing nosocomial infection is widely distributed clinically, which can be manifested as a variety of biological phenotypes, and its sensitivity to commonly used antibacterial drugs is also quite different. Therefore, clinical attention should be paid to pathogen examination in the diagnosis and treatment of infectious diseases.

    Relationship between lipoprotein-associated phospholipase A2 and ambulatory arterial stiffness index in patients with essential hypertension

    Objective

    To investigate the relationship between lipoprotein-associated phospholipase A2 (Lp-PLA2) and ambulatory arterial stiffness index (AASI) in patients with essential hypertension.

    Methods

    A total of 178 patients with essential hypertension hospitalized at Xuzhou Central Hospital from October 2018 to October 2019 were enrolled. AASI was calculated through 24-hour ambulatory blood pressure monitoring. According to the median value of AASI, the patients were divided to either a low AASI group (less than the AASI median; n=89) or a high AASI group (equal to or above the AASI median; n=89). Differences of age, gender, body mass index, smoking history, diabetes history, systolic blood pressure, diastolic blood pressure, fasting blood glucose, triglyceride (TG), total cholesterol (TC), low density lipoprotein-cholesterol (LDL-C), high density lipoprotein-cholesterol (HDL-C), estimated glomerular filtration rate (eGFR), uric acid, and Lp-PLA2 levels between the two groups were evaluated. The correlation between Lp-PLA2 and AASI was assessed. Multivariate linear regression analysis was performed to analyze the association of Lp-PLA2 and clinical data with AASI.

    Results

    Compared with the low AASI group, age [(67.87±14.23) years vs (55.65±14.59) years], systolic blood pressure [(136.96±16.23) mmHg vs (128.35±14.23) mmHg], fasting blood glucose [(5.95±2.26) mmol/L vs (5.36±0.82) mmol/L], and Lp-PLA2 [127 (100, 169) ng/ml vs 103 (86, 150) ng/ml] were significantly higher, and diastolic blood pressure [(72.26±12.87) mmHg vs (77.30±10.49) mmHg] and eGFR [(92.49±18.66) ml/(min?1.73 m2) vs (106.90±15.83) ml/(min?1.73 m2)] were significantly lower in the high AASI group (t=-5.654, P<0.001; t=-3.762, P<0.001; t=-2.296, P=0.024; Z=-2.470, P=0.014; t=2.867, P=0.005; t=5.553, P<0.001). Spearman correlation analysis showed a positive correlation between Lp-PLA2 and AASI (r=0.237, P<0.001). Multivariate linear regression analysis showed that systolic blood pressure (β=0.431, P<0.001), diastolic blood pressure (β=-0.371, P<0.001), fasting blood glucose (β=0.160, P=0.013), and Lp-PLA2 (β=0.152, P=0.022) were independent risk factors for AASI.

    Conclusion

    There is a positive correlation between Lp-PLA2 and AASI in patients with essential hypertension.

    Effect of proton pump inhibitors on image quality of magnetically controlled capsule endoscopy in elderly patients

    Objective

    To evaluate the effect of proton pump inhibitors (PPI) on cleanliness and visibility during magnetically controlled capsule endoscopy (MCE) in elderly patients.

    Methods

    Patients (≥60 years) who underwent MCE from January 2018 to April 2019 at Peking University First Hospital were included. They were divided into two groups based on PPI use or not before MCE. The quality of gastric preparation was assessed by cleanliness and visualization scores in six regions (cardia, fundus, body, angularis, antrum, and pylorus). Information on cleanliness and visualization scores, adverse events, and examination outcomes of MCE was collected and compared between the two groups. Cleanliness and visualization scores were compared using Mann-Whitney U test. Chi-square test or Fisher exact test was used for comparison of examination outcomes.

    Results

    Ninety-three patients (70 males; median age, 75.0 years; age range, 60-93 years) were included and divided into either group N (without PPI, n=54) or group P (with PPI, n=39). The total cleanliness score in group N was higher than that of group P [22.0 (6.0, 23.0) vs 19.5 (11.0, 23.0), Z=-3.186, P<0.001]. The total visualization score in group N was also higher than that of group P [18.0 (6.0, 18.0) vs 17.0 (11.0, 18.0), Z=-2.158, P=0.031]. Gastric preparation of group N was better than that of group P, especially at the fundus and body of the stomach. The rates of positive findings were 57.4% (31/54) and 51.3% (20/39) in groups N and P, respectively (χ2=0.343, P=0.558). No patients experienced capsule lodgment in the gastrointestinal tract. No adverse event was observed during the examination.

    Conclusion

    In elderly patients, PPI use appears to decrease the image quality of MCE. However, it does not affect examination outcomes. Current study suggests that MCE is overall a safe examination in the elderly.

    Influence of community group psychological intervention on mental and sleep health of elderly people

    Objective

    To investigate the effect of community group psychological intervention on the mental health and sleep health of the elderly, to provide a basis for improving the psychological and sleep health of elderly people.

    Methods

    A community group psychological intervention was carried out in a community of Beijin The psychological and sleep health of elderly people in the Kaiyangli community of Fengtai District, Beijing before and after community intervention was investigated from October 15, 2018 to January 2, 2019 using the self-rating anxiety scale (SAS), self-rating depression scale (SDS), and pittsburgh sleep questionnaire (PSQI). A total of 64 questionnaires were administered, and 58 were valid. The intervention was group psychological activities carried out by professional psychologists for the elderly in the community, once a week, for a total of 8 weeks, and each activity lasted 90 minutes.

    Results

    Before the intervention, the average SAS score of the elderly in the community was (47.10±1.03), and after group psychological intervention, the score was (43.60±1.03); there was a significant difference between before and after the intervention (t=3.358, P=0.001). The average SDS score [(53.16±1.47) vs (50.07±1.35), t=2.056, P=0.044] and average PSQI score [(6.16±0.52) vs (4.61±0.49), t=4.360, P<0.001] were also significantly lower after the intervention than before the intervention.

    Conclusion

    Group psychological intervention can not only improve the anxiety and depression of elderly people in communities, but also improve their sleep status, which is worthy of adoption and promotion.

    Hepatolithiasis is a type of cholelithiasis, also known as primary intrahepatic cholelithiasis. It refers to the stone that occurs in the bile duct above the left and right hepatic ducts and their confluence. Clinically, it can show a series of symptoms such as fever, abdominal pain, and jaundice. The stimulation and obstruction caused by stones will induce biliary cirrhosis, lead to cancer, and even endanger the life of patients. Due to the limitations of traditional laparotomy, minimally invasive treatment has been widely used in clinical practice to improve the quality of life of patients. This article give a brief review of the progress in the minimally invasive treatment of hepatolithiasis.

    Objective

    To make a comparative study of the value of automated breast volume scanner (ABVS) versus mammography (MG) in the diagnosis of breast neoplasms.

    Methods

    One hundred and twenty female patients with 156 breast neoplasms diagnosed by conventional ultrasound, ABVS, and MG at our hospital from May 2016 to December 2016 were included in this study. Using the pathological results as the gold standard, the diagnostic efficiencies of conventional ultrasound, ABVS, and MG were comparatively analyzed.

    Results

    The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of conventional ultrasound in the diagnosis of breast neoplasms were 79.5%, 72.6%, 76.3%, 76.7%, and 75.7%, respectively; the corresponding values were 97.6%, 87.7%, 92.9%, 90.0%, and 97. 0% for ABVS, and 80.7%, 74.0%, 77.6%, 77.9%, and 77.1% for MG. The sensitivity, specificity, and accuracy of ABVS were significantly higher than those of conventional ultrasound (χ2= 11.65, 4.30, and 15.39, respectively, P<0.05). The sensitivity and accuracy of ABVS were significantly higher than those of MG (χ2= 10.35 and 13.49, respectively, P< 0.05). The diagnosis of breast calcification by ABVS and MG was comparatively consistent (Kappa= 0.79, P< 0.05).

    Conclusion

    The specific coronal plane image of ABVS can clearly show breast neoplasms. Compared to MG, ABVS performs better in terms of sensitivity and accuracy.

    Chloasma is a disorder of symmetrical distribution of excessive facial melanosis, which usually occurs in middle-aged female and can affect their quality of life. The pathogenesis of chloasma is affected by many factors, including ultraviolet radiation, endocrine disorders, hormone levels change and genetic factors, etc. The pathogenesis remains unclear due to its complexity. There are various treatment methods for chloasma, especially laser treatment, whose accuracy and selectivity are reported to be excellent. Here the authors review relevant research of laser treatment in chloasma.
